She hates needles and maybe she wouldn't be able to handle a needle with low blood sugar, Im' the same way.
Fasting before a bloodtest will show accurate triglycerides, cholesterol and glucose levels.

Cholesterol is obviously not an issue for her and her triglycerides would be lowered if she fasted. I don't really think she has much to worry about. Do you think she's at any risk for coronary artery disease due to high triglycerides? Doesn't high triglycerides relate to coronary artery disease when LDL cholesterol is also high?
She doesn't eat carbs in excess so I'm sure she doesn't have to worry much about acurate glucose levels either especially if she is showing no symptoms and glucose levels are generally stabalized eating a low fat diet.
I think you can still tell a lot from a bloodtest without fasting, you just have to know what levels will be effected.
