Dough Graham did run into B12 problems and took supplements to fix things up. Later on he blamed it on stress. In his case I wouldn't believe anything much he said.

Some I would believe and trust like John Kohler. He went into 100% RV not for humanitarian purposes but for health reasons. At one point he went back to try cooked food but it didn't work so he went back to 100% raw, but a year or 2 ago he was a Beegun ( including bee products ). I don't know his current status.

It would be interesting for someone to follow the questionable ones around 24 / 7 for 3 months and see how they go in case they sneak in the odd jar of tahini ( as Harley claimed Doug ate ), peanut butter, roasted nuts or the occasional taco or baked potato. I believe some are genuinely 100% RV, but a lot would also depend on their definition of 100% raw.

When 60 minutes here in Oz followed Jasmuen the breatharian around she was a shonk.

I believe some people can be ok without B12, but they're like Charlie Smith and his sardines - very rare.
